For getting finance help for medical treatment people need to submit:

1.      Request (form completed by the municipality) with:

2.      Photocopy of ID card,

3.      Copy of a transaction account,

4.      Confirmation of employment,

5.      Solution that she/he is a beneficiary of social welfare.

The prepared Solution is delivered to the applicant by mail.

The deadline for adoption of the Act is 30 days after receipt of complete documentation.
