For getting financial aid for medical treatment people need to submit:

1.      Request (form completed by the municipality) with:

2.      Photocopy of ID card ,

3.      Copy of transaction account ,

4.      Confirmation of employment,

5.      Medical documentation,

6.      Solution that is a beneficiary of social welfare.

The prepared Solution is delivered to the applicant by mail.

The deadline for adoption of the Act is 60 days after receipt of complete documentation.
